Patrick C.
Forward Deployed Healthcare AI Engineer | EHR, FHIR, HIPAA LLM
Healthcare AI that actually reaches clinicians: I design and build it from scratch, integrate it with Epic or any EHR, validate it under HIPAA, and ship it. If your pilot is stuck at integration or go-live, I get it unstuck. I work on Epic, Oracle Health (Cerner), athenahealth, eClinicalWorks, NextGen, Medplum, MeldRx, or any FHIR R4 or HL7 v2 endpoint. In the last 14 months I shipped three production AI applications integrated with live EHRs under HIPAA: a family-facing AI assistant on Epic Patient Access FHIR for Baylor College of Medicine / Texas Children's Hospital (AWS Bedrock, pgvector RAG over clinical notes, 29 versioned clinical-setting prompts); a CDS Hooks and SMART on FHIR emergency observation platform for Kineta Health on the ePowerDocs EHR via MeldRx, with a Medplum demo environment, live at its first clinical site; and SMART on FHIR, MCP server, and International Patient Summary tooling for The Commons Project / JupyterHealth with UC Berkeley and the University of Chicago, all open source on GitHub. What I do • EHR integration across vendors: SMART on FHIR patient and provider launch, CDS Hooks, FHIR R4 and Bulk FHIR, HL7 v2 and C-CDA interfaces, Epic Patient Access, Medplum, MeldRx, HIE and TEFCA connectivity • HIPAA-scoped LLM and RAG architecture under a BAA: Claude, OpenAI, AWS Bedrock, MCP servers, structured clinical output • Evaluation and monitoring harnesses with clinician-written rubrics, mapped to Joint Commission and CHAI responsible-use guidance • Cloud platform engineering on AWS and GCP: ECS/Fargate, Lambda, RDS, Secrets Manager, Cloud Run, CI/CD, cost consolidation, alarms and budgets Why it lands: I practiced emergency medicine for 18 years as a physician assistant, served as CMO, CTO, and CMIO of a health technology company where I led HL7, API, and RPA integrations with every major US EHR and practice management vendor, and lead engineering and AI governance for PHI at a healthcare nonprofit alongside its security and compliance team. I speak CMIO, compliance, and engineering in the same meeting, which is where most AI pilots die. How I work: one senior engineer, embedded with your team, owning the outcome from scoping to go-live. No bench, no handoffs, no markup. A fraction of the cost of a consultancy pod or a full-time forward deployed hire. Accepting new engagements, remote, with on-site by arrangement. I sign BAAs, carry cyber and general liability insurance, and can provide clinical and academic references. "Mr. Carter did phenomenal work on this project. He successfully built a complex Epic integration." (Upwork client, 2025) Engagement options: hourly builds, monthly embedded retainers, and fixed-scope diagnostics (EHR/FHIR integration readiness, HIPAA LLM architecture review).
